Heavens Peak Health Care is teaming with Berube PT for a sports physical clinic for those who missed the one in June (which was huge success).

Cost is $20.00 cash or check only (checks payable to CFHS)Clinic will be Aug 10th from 5:30-8PM.
We request parent be present or note for permission to be seen.
Money will be donated to the CFHS athletic program as was done last year.
Clinic will be at Heavens Peak Healthcare in CF.
